The Report is quantitative. Your markers — from standard panels to functional lab testing — are read against optimal ranges, not just conventional ones. Your results are plotted over time, including labs imported from previous providers and from at-home or membership testing services, so trends speak before symptoms do. Treatments are tracked against outcomes: what was tried, when, and what your numbers did in response. And because the findings come with their sources, you can see where the conclusions come from.

A $50 deposit holds any appointment. We accept HSA and FSA cards, and provide superbills you can submit for possible out-of-network reimbursement. Labs and supplements are billed separately. The Report is offered with Dr. Jacob’s visits, pediatric included.

Care-package members: your package pricing doesn’t change. Add the Report to any individual package follow-up with Dr. Jacob for $250, selected when you book or arranged at your visit.

What exactly is in the Report?
A written, data-driven document built around your numbers: your labs read against optimal ranges, your results from different providers and prior years synthesized into one place, your results laid out over time, and how your symptoms, history, and numbers fit together — with research citations. Because it’s built from your data, its shape follows your story rather than a fixed template, so no two Reports read quite alike. The fastest way to understand it is to see a sample — we’ll send one over.
Is the visit different if I add the Report?
No — the visit is identical in length and depth, and root-cause analysis happens in the room either way. The Report is the additional synthesis and writing Dr. Jacob does with your data after you leave: pulling your results together, laying them out across visits, and documenting the whole picture in one portable PDF.
How do I add the Report to my visit?
Book your visit with Dr. Jacob as usual on our scheduling page, then select the Panka Functional Health Report add-on before checkout — a flat $250 with any visit.
I already have results from an at-home or membership lab service. Can you help me make sense of them?
Yes — this is exactly what the Report is built for. Bring results from any testing service to your visit: Dr. Jacob reviews outside labs as part of every appointment, and the Report gathers them into one place, read against optimal ranges, with citations. Testing finds the data; the Report helps make sense of what it shows.
I wear a CGM. Can that be part of my Report?
Yes. Bring your continuous glucose monitor data and Dr. Panka can review it in detail as part of your Report — a deeper look than the daily graph and averages your app displays, into what your glucose is really doing around meals, sleep, and stress. It's one of the spots patients are most often surprised, finding meaning in data they thought they already understood.
Can I share the Report with my other doctors?
Yes, and many patients do. It’s your document, written in plain language with citations, so your PCP or specialist can follow it and see the sources. It’s designed to travel with you across your care.
Why one flat price for every Report?
Because the work is substantial either way. New-patient Reports carry records review and baseline building; follow-up Reports document your results across visits and what changed in response to treatment. Rather than slice it, we kept it simple: $250 per Report, any visit.
I have a long, complicated health history. Will the Report cover all of it?
The Report works from your current visit, your labs, and the key trends in the records you share. For long or complex histories, the longitudinal section synthesizes what matters most — it’s a clinical document, not a page-by-page chart review.
Is the Report available for pediatric visits?
Yes — any of Dr. Jacob’s visits can add the Report, children included. An honest note: younger patients’ histories are often simpler, and many families won’t need this depth. The choice is yours, and Dr. Jacob will say so plainly if he doesn’t think a Report would genuinely add value for your child. Our pediatric care itself is unchanged.
When and how do I receive it?
It comes as a secure PDF, either by secure message in your Charm patient portal or by encrypted email — your preference.
Do you take insurance?
We’re a direct-pay practice — naturopathic doctors in Minnesota are out-of-network with all insurance plans. We provide superbills for possible out-of-network reimbursement and accept HSA and FSA cards.
Is this a membership or subscription?
No. No membership, no monthly fee — that’s how we work. The Report is a per-visit choice, never a commitment.
