Schedule Your Appointment
Choose between in-person or virtual appointments
After scheduling, you'll receive an email from the Charm Patient Portal (our electronic health record system) within 24-48 hours on business days (check your spam folder if you don't see it).
Virtual appointments are available to patients located in Minnesota or Wisconsin at the time of the visit. If you'll be out of state, please let us know in advance
Complete Required Paperwork
Create your Charm account using the email link you receive
Fill out our new patient questionnaire online before your appointment
Log in to the Charm patient portal
Click 'Questionnaires' on the Home screen and complete the form online
Estimated completion time: 20-25 minutes.

Share Your Medical Records
Important: Upload and ‘Share’ pertinent health records with your provider in Charm
What to share: Recent labs (within the last year), imaging reports, current medication list, relevant specialist notes, recent hospitalization or surgical reports
Log in to your Charm patient portal
Go to the 'Documents' section
Upload your documents (accepted formats: .png, .jpeg, .pdf, .doc under 50 MB)
After uploading, select each document, click ‘Share’ and choose your provider's name. This final sharing step ensures your provider can access your records
As an alternative method, you may request a 'Release of Protected Health Information' from your other provider(s) and have them fax your medical records to us at 612.568.9020.
Payment Information
Booking & Deposit
When you schedule online through our scheduling platform, a $50 deposit is collected at the time of booking and applied toward the cost of your visit.
Your card information is securely stored on file for future charges, including services, supplements, lab tests, and any applicable late-cancellation fees.
If you schedule by phone, a deposit may be collected at booking — payment for your visit is handled at the time of service.
Payment Options
Any remaining balance is due at the time of service
We accept:
Cash, check, or bank transfer
Major credit cards (Visa, MasterCard, American Express, Discover, JCB, UnionPay)
HSA/FSA cards - though some transactions are subject to your card administrator's approval
Insurance
We operate as a cash-pay practice and do not contract with health insurance. We can provide superbills you may submit to your insurer for possible reimbursement, but we cannot guarantee any plan will reimburse
Medicare and Medicaid will not reimburse for our services.

Entrance is on the street side of Suite 901
Please note: Our office entrance is reached via four steps with handrails on both sides, plus a small step at the building door. The office is not wheelchair accessible. If stairs are a barrier, please contact us before scheduling — virtual appointments are an excellent alternative, and we're happy to discuss options that work for your needs.

Please plan to arrive 5–10 minutes early for your first visit
If possible, bring pictures or the actual bottles of your current supplements and medications
Virtual Appointments
How to Join Your Virtual Appointment:
Find the URL link in your appointment confirmation email (next to "Where")
At your scheduled time, click this link and select "Ask to Join"
We use HIPAA-compliant video conferencing
Please be in a private, quiet location where you can speak freely. You should be located in Minnesota or Wisconsin during your appointment — if you'll be elsewhere, contact us in advance
Your provider will admit you when ready to begin
Ensure your camera and sound are turned on
If You Experience Technical Issues:
Call our office at 612.568.8382 for assistance
If you haven't joined within 15 minutes of your start time, your provider will call you as a courtesy
Please allow a few minutes if your provider isn't immediately available, as they may be finishing with another patient
At Your Visit
To keep our attention on you rather than on note-taking, we use a HIPAA-compliant scribe tool that converts our conversation into clinical notes in real time. The system has strict privacy safeguards and all information remains within your confidential medical record. If you'd prefer we not use it during your appointment, just let our staff know before your visit or tell your provider at any time.
Communicating With Us as a Patient
Staying in touch between visits is part of your care — included, with no extra fee and no membership, however long it's been since your last appointment.
Here are the best ways to reach us:
• Charm Patient Portal secure messaging is the right channel for non-urgent clinical questions, supplement questions, lab questions, and most ongoing communication with your provider. Messages go directly to your provider and are stored securely with your health record. Log in at pankanaturalhealth.com/patient-portal.
• Phone (612.568.8382) is a fully legitimate option if the portal doesn't work well for you, particularly if you're not comfortable with online tools. Leave a detailed voicemail and we aim to return calls by the next business day; as a small doctor-led practice, callbacks may take longer during busy weeks.
• A follow-up visit is the best option for new or changing health concerns, and for substantive clinical questions that need real time and attention. If a stretch of time goes by between visits, your provider may suggest a follow-up to keep your plan current. Book at our scheduling page.
• info@pankanaturalhealth.com is best for administrative matters like billing, scheduling logistics, or document requests. For clinical questions, please use Charm so they're stored with your medical record.
For medical emergencies, please call 911 or go to the nearest emergency room. Our practice isn't equipped for same-day or acute care needs.
Cancellation Policy
24-hour notice required for cancellations or rescheduling
Please cancel or reschedule through your Acuity Scheduling confirmation email — look for the "Change Appointment" or "Cancel Appointment" link near the bottom of the email
$50 fee for no-shows or late cancellations (within 24 hours)
If you can't find your confirmation email or need help rescheduling, you can also email info@pankanaturalhealth.com or send a message through your Charm portal
We understand emergencies happen; reach out if circumstances are unusual — exceptions may be
made at the provider's discretion
